Our Community Health Occupational Therapists (OTs) in Norwich and Social Services Occupational Therapists from Norfolk County Council (NCC) work together to triage OT referrals from a joint SystemOne OT waiting list. OTs from NHC&C and NCC attend joint training and workshops to improve the service we provide.
The benefits of this have been:
- Reduction in duplicated referrals through a joint triage approach
- Deployment of the most appropriate OT with the right skill set
- No more duplication of a social care OT/health OT home visits
- OTs seek advice from colleagues rather than creating a new referral which has resulted in greater continuity of care
- Patients only have to tell their story once
- Staff now have extended skills and a better understanding of health/social care OT roles
