“Each day, I conduct Discharge to Assess (D2A) Therapy assessments, ensuring patients receive the right support after leaving hospital. It’s more than just mobility checks—I assess their overall well-being, including skin condition, vital signs, and home environment.

I also respond to Admission Avoidance (AA) cases, which require quick thinking and adaptability. These patients often come with limited medical history, so we must assess their situation rapidly and ensure they’re safe. I enjoy the problem-solving aspect of this work.

What makes this role even better is the autonomy—we manage our own caseloads while driving through the stunning Norfolk countryside. And yes, I may belt out a power ballad or two between visits! The team is always supportive, and help is just a phone call away.”

Mary’s story: Community Assistant Practitioner in North Urgent Care Response

“My day starts with reviewing my patient list and liaising with colleagues. I then set off across North Norfolk, meeting people in their homes to assess their needs and help them work towards their goals.

Some visits involve ordering equipment like grab rails or hospital beds, while others require hands-on support with mobility exercises. There’s no better feeling than seeing a patient progress—sometimes I even cheer when they reach a milestone!

Beyond physical support, we also handle wound care and health monitoring, ensuring a holistic approach to patient well-being. This role is truly rewarding.”
